This is an absolute deadline. Any students who do not have a consent form on file in the Nurse’s Office by this deadline will not be vaccinated.
Appointments will be scheduled once the consent form is received. Walk-ins (students coming in on the date of the clinic with a signed consent form) will not be permitted (because UCFS will not have brought a vaccine).
