Middle School Vaccination Clinics
FORMS FOR AGES 12 TO 15-YEAR-OLDS
As you may know, this week the FDA (U.S. Food and Drug Administration) is expected to allow the Pfizer COVID-19 Vaccine to be administered to children 12-15 years of age. In anticipation of that authorization, Norwich Public Schools is pleased to partner with UCFS Healthcare to offer middle school students COVID-19 vaccination clinics.
Please print, fill in and send back to your child’s school.
Consent Forms DUE Friday, May 14, 2021, at 2 PM.

Kelly STEAM Magnet Middle School:
The first Pfizer dose will be administered Monday, May 17, starting at 8:30 am and the second dose will be administered Monday, June 7, also starting at 8:30 am. Kelly parents can return the consent form to the school or email it to Nurse Jajai Quinn at jquinn@norwichpublicschools.org.

Teachers Memorial Global Studies Magnet Middle School:
The first Pfizer dose will be administered Tuesday, May 18, starting at 8:30 am and the second dose will be administered Tuesday, June 8, also starting at 8:30 am. Global parents can return the consent form to the school or email it to Nurse Margaret Shieber at mshieber@norwichpublicschools.org.

Those eligible to participate in this clinic include:
Any NPS student age 12-15 with signed parent consent

If you are interested in having your child participating in this free clinic, please fill out this form and return it to your school nurse by 2 pm on Friday, May 14, 2021. This is an absolute deadline. Any students who do not have a consent form on file in the Nurse’s Office by this deadline will not be vaccinated.

Appointments will be scheduled once the consent form is received. Walk-ins (students coming in on the date of the clinic with a signed consent form) will not be permitted (because UCFS will not have brought a vaccine). There is no make-up day so if your child is absent they will not participate in this clinic.

The vaccination clinic is open to in-person and remote learners.

If you have any questions please call the school principal.
